A radio button rating question is a type of rating question, where respondents can choose an answer between 0 and 10. Only a single answer can be chosen.
Rating questions are question types that ask a respondent to rate a (part of a) product or service. There a lot of different ways to ask someone to rate your product. Check out our rating question guide if you want to use a visual rating scale such as stars or emojis.
The answer buttons from 0 to 10 are already set up for you and will look like this:

1. Add a radio button rating question
In the Questions tab, click the button ?Questions. Select Radio Button Rating from the list of question types and click Add.
2. Change the general question settings
- Hide/show the question settings.
- Change the question order.
- Change question type.
- Make the question active/inactive.
- Lock/unlock the question to be edited by your team members.
- Copy, Delete, or Preview that particular question.
3. Edit the question
- Add your question here. Use the rich text editor icons to make your text bigger, add emojis or videos, change the alignment, and more!
- Click Add media or Add YouTube video to upload images or videos to make your questionnaire more entertaining. The file needs to be under 2MB. To add a YouTube video, paste a YouTube link after you've clicked the Add YouTube video button.
- Make the question mandatory - toggle the button to require respondents to answer the question before moving to the next question.
- Add a question tag - turn this feature to add a tag to your question that you can reference in your survey or PDF.
4. Change answer options
- Add a 'not applicable'-answer - toggle this button and an answer option Not applicable will be automatically shown at the bottom of the answer options.
- Add a text box to specify or leave an extra comment - When this feature is switched on, you'll be able to add your own text to ask respondents to give more information.
- Add button to upload an extra image file - This feature allows the respondents to upload their own image files. You can use this feature to make the survey more engaging.
5. Benefits & Usage
Radio button rating benefits:
- By including the 0 points on the scale, it is clear to respondents that 0 must be the lowest rating. When using 1-10, confusion can arise whether 1 means the top or the bottom. Avoid this confusion by simply inserting this question which comes with 0 to 10 numbered automatically.
- The midpoint of the scale is 5, leaving 5 options below the midpoint and 5 options above. This helps the respondent make a decision, and makes analysis easier as well.

- Ask employees to rate a training session.
- Check how well students feel they understood a lecture.

- Image Choice is used when you ask a multiple choice question and present the possible answers to your respondent by using images or GIFs. Photos in your questionnaire allow you to connect with your audience on a more emotional level and it’s proven that people often respond better to a picture.
- Text Choice is the simplest question type where you ask your respondents to choose the right answer. It can also be turned into a quiz question if you add a score or into a multiple choice question. In that case your respondent is asked to choose one or more items from a limited list of choices.
Text Only allows you to add a screen or slide with only text (and a media file) in your questionnaire. It will add interaction and personality to your questionnaire because instead of only asking for information, you have the opportunity to give information as well. It helps to create a dialogue and make your questionnaire feel interactive.
